Coastal Carolinas Health Alliance (CCHA) is a network of 11 hospitals in 11 counties throughout southeastern North Carolina and Northeastern South Carolina.

The Community Health Improvement Team comprises representatives from each of the eleven hospital members of CCHA. These participants work closely together and with other healthcare providers in the region to provide the residents in their community with the most up-to-date diabetes resource information in the county.

The team began with 8 hospital members in 2001 launching a Diabetes Prevention and Awareness Campaign under a Duke Endowment Grant. Over the subsequent years the team has grown to its present size. The team continues to launch initiatives that benefit our region even beyond the life of the grant:
